Licensed Practical Nurse

We are seeking dedicated and experienced Licensed Practical Nurses (LPN) to join our team. The ideal candidate will provide comprehensive nursing care to patients in their homes, ensuring they receive the highest standard of care. You will be responsible for administering medications and collaborating with other healthcare professionals to optimize patient outcomes. 

• Provide direct patient care in accordance with the care plan.
• Assist with the development and implementation of individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patients' conditions and report any changes to the supervising nurse or physician.
• Educate patients and their families on disease management, treatment plans, and preventive healthcare practices.
• Document patient care activities accurately and timely.
• Coordinate with physicians, registered nurses, and other healthcare professionals to ensure comprehensive patient care.
• Provide compassionate support and comfort to patients and their families.
