Hospice Registered Nurse

We are seeking dedicated and compassionate Hospice Registered Nurses (RN) to join our team. The ideal candidate will provide expert care and support to terminally ill patients, focusing on pain and symptom management. You will work closely with the hospice care team to develop and implement personalized care plans that honor the patient’s wishes and provide emotional and practical support to families.

• Conduct comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
• Develop, implement, and evaluate individualized hospice care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments to manage pain and other symptoms effectively.
• Provide education and support to patients and their families on disease progression, symptom management, and end-of-life care.
• Monitor and document patients’ conditions, progress, and any changes, ensuring accurate and timely communication with the care team.
• Coordinate care with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive patient care.
• Offer emotional support and counseling to patients and their families, helping them navigate end-of-life decisions and cope with grief.
• Ensure compliance with all regulatory and accreditation standards related to hospice care.
